Purpose of the Position

To implement and monitor the Case Management Model (CMM) by providing adherence support to ART patients until they are eligible for decanting to differentiated models of care.


Minimum Requirements

  • Grade 12 certificate
  • Enrolled Nurse qualification (added advantage)
  • Minimum of 2 years’ relevant experience in counselling and testing
  • Experience in adherence counselling
  • Basic HIV testing and counselling
  • Index case tracing
  • Ability to communicate in English (isiXhosa is an advantage)
  • Criminal clear record


Duties and Responsibilities

  • Capture details of newly initiated ART patients and those on ART for less than one year on the CMM Excel spreadsheet and track appointments
  • Record clients with high viral loads, paediatric clients, pre-ART clients, and pregnant women
  • Contact clients who miss appointments and link them to CHWs or appropriate community support
  • Conduct home visits for missed appointments, unsuppressed patients, and those requiring additional care
  • Send routine SMS appointment reminders
  • Respond professionally to client messages
  • Refer patients to support groups based on their preferences
